Abstract: Congenital Protein C deficiency is a rare inherited thrombophilia with autosomal dominant inheritance. Bilateral lower limb ischemia and gangrene is a rare complication of Protein C deficiency. Index case presented with bilateral symmetrical lower limb gangrene, bilateral cataract with non palpable femoral pulse. USG Doppler showed aortic thrombus and coagulation profile revealed deficiency of Protein-C. Parents also showed Protein-C deficiency on further workup. Patient was managed with low molecular weight heparin and supportive care.
